I work as a Registered Nurse at the Medical Rehabilitation Center under the Public Authority of Disabled Affairs .

PSYCHIATRIC & MENTAL HEALTH DEPARTMENT (REHABILITATIVE CARE)

My duties and responsibility are to provide comprehensive care to the residents, who permanently reside at our center and have various diseases and disorders. These residents are bedridden or wheelchair-bound due to medical and neurological issues, and they require complete nursing care for their daily activities.

In terms of the daily schedule, I have three shifts.

• At the beginning of my shift, I have to check psychotropic stock and issue medication as per order and also ensure that emergency facilities as well as inventory in order.

• Secondly, I receive endorsement from the previous shift, which includes information about the residents' conditions, any changes in medication, investigations reports, diagnostic evaluations, etc. Then I conduct a bedside handover for each resident.

• Subsequently, I begin my routine work based on the assigned tasks which include observing the personal hygiene of the residents like bath, dressing, mouth care, and grooming which are performed by assistant nurses.

• I also provide wound care for patients with tracheostomy, gastrostomy, and colostomy.

• Additionally, I provide necessary care for patients who require BIPAP support.

• Emergency management of patients with seizures, choking, falls, and cardiac arrest by providing CPR, ECG monitoring, oxygen administration, suction, first aid, etc.

• Other responsibilities include:

• Monitoring and recording patient vital signs and reporting to the physician if there are any variations in the reading.

• Conduct physical examination and report and record any significant signs and symptoms.

• Administration of medication to the residents by following the ten rights of medication administration.

• Feeding for residents by oral/ Nasogastric tube feeding/ PEG feed via a feeding pump.

• Provide a safe and healthy environment for the residents.

• Attend doctor's rounds and carry out doctor's orders.

• Cooperate and coordinate with healthcare team members to prepare effective patient care plans.

• Carry out sample collection for laboratory investigation and other tests, and diagnostic procedures, evaluate the results, and inform the doctor.

• Collaboration with various departments such as laboratory, physiotherapy, biomedical, housekeeping, laundry, kitchen, and pharmacy to provide comprehensive care to the residents.

• Providing assurance and clearing the doubts of family members when they visit the patients.

• Training and monitoring junior staff.

• Documentation of reports, records, and patients' file

• Maintain stock and check the availability of supplies accordingly.

• Follow infection control policy to prevent infection among residents and report any infection cases to the infection control unit.

• Attend in-service education programme to update the knowledge.

• Write incident report in case of any unusual incidents

CCU

• Nursing care of clients with cardiac problems.

• Preparing clients for procedures such as angiography.

• Competent in providing cardiopulmonary resuscitation.

• Monitoring of Arterial Blood Gas Analysis.

• Preparation and administration of medicines, injections and intravenous infusion of blood and blood products.

• Preparing and assisting for Ryle's tube insertions, suture removal and pacing wire removal.

• Supporting clients with attending to their personal hygiene.

• Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.

• Promote patients' independence by establishing patient care goals, health education of patients and their significant others and self-care skills.

• Assures quality of care by adhering to therapeutic standards, measuring health outcomes against patient care goals and standards

• Resolves patient problems and needs by utilizing multidisciplinary team strategies.

• Maintains safe and clean working environment by complying with procedures and policies within the working environment.

• Evaluation of care provided and modifying the plan based on attainment of goals.

• Maintaining proper inventories.

• Maintaining accurate records utilizing information technology systems

• Coordinating admissions, discharges, and transfers.

• Competent with use of various equipments such as IABP Machine , defibrillator, ventilator, Holter Monitor , cardiac monitor and syringe pumps.

• Maintains continuity of care by documenting and communicating care in a timely manner.

• Maintains patient privacy.

• Maintains collaborative relationship with the health care team members by communicating information, responding to requests, building rapport, and participating in team problem-solving.

• Following good infection control, food hygiene, moving and handling, medication awareness, health and safety rules and regulations to provide maximum quality care for the service users.

• Assisted with exercises, physiotherapy and other medical plans.

• Managed any unanticipated events or unstable situations.

• Monitored supplies and resources.

• Organized induction training for new staff.

SURGICAL WARD

• Pre-operative and post-operative nursing care of clients undergoing various surgeries

• Nursing assessment of clients.

• Preparing clients for their respective surgeries.

• Monitoring of vital signs

• Hot and Cold Application E.g., Ice Bed, Hot Water Bag, Cold Compress, Cold Sponge

• Dressings and wound care, oxygen therapy, nebulization

• Assisting in bed side procedures.

• Supporting clients with attending to their personal hygiene.

• Preparation and administration of medicines, injections and intravenous infusion of blood and blood products.

• Maintaining aseptic techniques.

• Proper disposal of biological and non-biological waste.

• Evaluation of care provided and modifying the plan based on attainment of goals.

• Maintaining proper inventories.

• Maintaining accurate records utilizing information technology systems

• Coordinating admissions, discharges, and transfers.

MEDICAL WARD

• Assessing, observing, and speaking to patients.

• Recording details and symptoms of patients medical history and current health.

• Preparing patients for examinations and treatments.

• Administering medications and treatments, then monitoring patient for side-effects and reactions.

• Creating, implementing and evaluating patients care plans with the medical team.

ICCU

• Nursing care of clients with cardiac problems.

• Competent in providing cardiopulmonary resuscitation.

• Preparation and administration of medicines, injections, intravenous infusion of blood and blood products.

• Preparing and assisting for Ryle's tube insertions and suture removal.

• Supporting clients with attending to their personal hygiene.
